wellness check ups for our teenage patients

Wellness for Our Tweens and Teens

We follow the American Academy of Pediatrics (AAP) recommended well check schedule.  At each check up we ask that the parent complete the age-appropriate Bright Futures questionnaire from the AAP.  For patients 11 years and older there are separate questionnaires for both the parent/guardian as well as the patient.  This screening questionnaire should be completed strictly by the patient confidentially.  The AAP recommends providers have time to speak alone to each patient starting in the teens.  This supports the provider/patient relationship and helps the patient learn how to access medical care.

Patients 18-21 years old

A patient that is 18 years and older can complete a HIPAA Patient Consent form and can consent to release information to individuals or not.  The law requires us to protect the privacy of our patients and we will not disclose medical information if a patient has declined to allow parents access to their medical information.

4 Reasons Teens Should Get Privacy at Checkups

Dr. Nancy Zises from the Palo Alto Medical Foundation has shared her thoughts on why it's important for teen aged patients to get time alone with their provider during a checkup.  Click here to read an article she has written about her ideas.
