If you are seeking care for more than one child, please complete a form for each potential patient.

New Patient Intake Form 

The office will review the form and determine what follow up questions or concerns need to be addressed.  Due to an overwhelming influx of new patient requests, there may be a delay in processing paperwork so we ask that you please be patient.  Once it is determined that our practice will be a good fit for your family, a member of our office staff will reach out to you for next steps.

You must verify that we are in network and participate with your insurance plan and we must be selected as your child's PCP. It is your responsibility to know the health benefits that your insurance provides and what services may or may not be covered. You will be required to provide proof of insurance coverage and a picture ID prior to your first visit.

If your child has been seeing a different pediatrician we will need to gain access to these records in order to have a coordinated effort. Contact them for a release form (a signed release will be required) and understand that there may be a charge for the service. Try to have at least an immunization record in hand or sent to us before your first visit with us.

We will obtain vaccine records that have been reported to the state through the Immtrac2 system. Please make sure to review our vaccination policy as we require all of our patients to be fully vaccinated and agree to follow all age appropriate immunization schedules. 

If your insurance is an HMO,  you will need to ensure one of our physicians is added as the primary care provider for the child and that we are in-network with your health plan.  Not doing so could result in unforeseen out of pocket expenses that will be personal responsibility.  If your child is covered by a Medicaid insurance plan, please make sure that they are added to our patient panel be selecting us as the child's PCP.

Once we have completed all the necessary documentation above, we will work with you to get your child set up with his/her initial appointment.
