Patient Portal

We are excited to bring you our new patient portal – an easy to use, comprehensive patient engagement tool brought to you by our electronic health record (EHR) partner athenahealth.

Through the convenient patient portal, you can schedule appointments, access appointment forms, see test results, request a prescription refill, find immunization histories, download patient visit summaries, make account balance payments and send messages to the clinic.  The portal can be accessed from a variety of devices such as tablets, laptops, mobile phones and desktops.

How to Register

Creating an account requires 2 easy steps.

  1. Provide your email address to our staff when you call to make an appointment or are in the office for a visit. 
  2. Follow the instructions in the email that you receive to set up your account.

Our office staff can also assist with getting you registered over the phone or in the office.  You will be provided with a temporary password that you will be required to change when registering.

You can also request an account by visiting the patient portal URL.  Click on the hyperlink that says “Sign up” today and provide us with the requested information.  Our staff will process your request and you will receive the registration email.

All established patients will be required to create a new portal account through athenahealth.  The patient portal supported by our previous EHR partner will no longer be accessible to either the patient or the provider.  You may need to provide us with your email address again in order to register successfully.
